Our Fourth Trimester Plan is how we care for all newborns during their first three months of life.
We believe the earliest weeks with your baby should be spent at home - resting, recovering, bonding, and settling into your new rhythm as a family. Instead of packing up your newborn for frequent office visits, your Holistic Home Pediatrics provider comes to you. You'll receive personalized, comprehensive pediatric care from the comfort of your home during your baby's entire fourth trimester.
Many families discover they love the convenience, personalized care, and peace of mind that home visits provide. While some continue with our home-based pediatric membership after three months, this plan also offers the opportunity to experience our approach before deciding on long-term care.
Please note: Our Fourth Trimester Plan is our standard newborn care option for babies from birth through 3 months of age. If your baby will remain under the care of a midwife for the first six weeks of life, please see the All Inclusive Monthly Plan or Fee For Service Plans.
What is the Fourth Trimester?
A term first coined by Dr. Harvey Karp, a pediatrician, refers to the transitional time between the birth of a baby and three months of age. Karp notes that babies are not yet fully developed, that it takes an additional three months for their central nervous systems to mature to the point of being able to really interact with the world around them. Karp notes that during this time a baby's primary needs are to be, held, fed, rocked, shushed and swaddled because crying is their only mode of communicating. Likewise, for parents this is a time of sleep deprivation, rapid hormonal shifts and of need for extra support.
All of the above has lead to increasing evidence that post-partum care of baby and Mom should be done in the home. Doing this leads to less re-hospitalization, reduced need for ER or urgent care visits, and fosters the development of a strong parent-baby bond. Our fourth trimester plan is designed with all of this in mind.
Services Provided
Prenatal consultation via Zoom, to discuss any relevant history, birth plan and your thoughts about your fourth trimester. Partners are welcome and encouraged to attend.
First newborn visit within 24-48 hours of birth for home birth and of hospital discharge for hospital births
Weight checks as needed during first two weeks of life (including collection of the second state newborn screening blood spot test)
One month and two month well checks
Transition back to work plan for Mom
Help transitioning to your child's pediatrician for the four month well check or to one of the other HHP plans
Telemedicine visits as needed
Electronic patient portal for each child
Access to primary provider via text, email, and telemedicine between visits to your home, and after normal business hours between 7am-11pm 7 days a week
Prescriptions when needed
Referral to specialists when needed

Membership Plan
This plan is ideal for families who want the highest level of personalized care and access to their pediatric provider. It includes all routine and sick visits, along with direct texting access to your nurse practitioner during extended hours.
Many families choose this plan during their baby's first year of life, when questions and visits tend to be more frequent. It's also an excellent option for children who need additional support or ongoing management of chronic health conditions, giving families the confidence of knowing their provider is just a text or visit away.
Services Provided
Well child checks based on AAP schedule: (2 days, 1 month, 2 months, 4 months, 6 months, 9 months, 12 months, 15 months, 18 months, 24 months, 30 months, 36 months then yearly for ages 3 years and above)
Longer visits (well child visits are usually 45-60 minutes long)
Follow- up for chronic conditions (Allergies, asthma, ADHD medication, eczema, etc.)
Tele-medicine sick visits
Sick visits (same day or next day)
Limited rapid diagnostic tests (Rapid strep, rapid flu, rapid RSV, rapid covid, Urinalysis)

This plan is ideal for families who prefer to pay for care as they go and don't anticipate needing frequent visits throughout the year.
Even with a pay-per-visit model, you'll still enjoy the longer appointment times and personalized, relationship-based care that Holistic Home Pediatrics is known for. You'll also have access to secure portal communication with your provider for questions and follow-up between visits.

Follow-up visit $180 (45 min) - for ongoing chronic conditions such as asthma, ADHD, eczema, allergies, etc...
