Fourth Trimester Plan

This plan is designed for families welcoming a newborn who want comprehensive, personalized pediatric care in the comfort of their own home during their baby's first three months of life.

The Fourth Trimester Plan provides all-inclusive newborn care, including routine well-child visits, weight checks, and extended direct access to your pediatric provider, allowing your family to focus on resting, recovering, bonding, and adjusting to life with your new baby.

It's an excellent option for families who value the convenience and peace of mind of home-based pediatric care during the newborn stage, with the flexibility to transition into another Holistic Home Pediatrics membership after three months.

Services Provided

  • Prenatal consultation via Zoom to discuss relevant medical history, birth plans, and newborn care expectations

  • First newborn home visit within 24–48 hours of birth or hospital discharge

  • Weight checks as needed during the first two weeks of life

  • Collection of the second state newborn screening blood spot test

  • Comprehensive 1-month and 2-month well-child visits

  • Personalized guidance on feeding, growth, development, and newborn care

  • Support with transitioning back to work and establishing feeding routines

  • Telemedicine visits when appropriate

  • Electronic patient portal for each child

  • Direct provider communication via text and email, including extended hours from 7 AM–11 PM, seven days a week

  • Prescriptions when needed, following provider evaluation

  • Referrals to specialists when needed

  • Guidance transitioning

Fourth Trimester Plan Pricing

$850 one-time, all-inclusive fee

$150 one time only family enrollment fee

The enrollment fee is due at the time of enrollment to reserve your baby's placement. The all-inclusive plan fee is due by the first newborn visit.

Additional Information

  • This plan covers newborn care from birth through 3 months of age.

  • All routine newborn visits included in the plan are provided in the comfort of your home.

  • Families may transition to our All-Inclusive Membership (AIM) or The Basics Membership after completing the Fourth Trimester Plan.

  • Families whose newborns remain under midwife care for the first six weeks may be eligible to begin care through another membership option, subject to provider approval.
