This plan is ideal for families who prefer the flexibility of paying for pediatric care as needed, without committing to a monthly membership.

The Fee for Service Plan provides personalized, relationship-based pediatric care with extended appointment times and individualized attention, allowing families to schedule routine wellness visits, sick visits, and follow-up care when needed.

It's an excellent option for families who anticipate fewer visits throughout the year but still value the thoughtful, comprehensive approach to pediatric care that Holistic Home Pediatrics is known for.

Services Provided

  • Comprehensive well-child visits based on the AAP recommended schedule

  • Sick visits for acute illnesses and concerns

  • Follow-up care for chronic conditions such as allergies, asthma, ADHD, and eczema

  • Extended well-child visits, typically 45–60 minutes

  • Comprehensive growth and developmental assessments

  • Personalized guidance on nutrition, development, and preventative health

  • Telemedicine visits when appropriate

  • Electronic patient portal for each child

  • Provider communication via patient portal and email during regular business hours, Monday–Friday

  • Prescriptions when needed, following provider evaluation (medication costs are the patient's responsibility)

  • Referrals to specialists when needed

  • Electronic medical forms

Fee for Service Visit Pricing

  • Well-Child Visit: $240 (45–60 minutes)

  • Additional Sibling Well-Child Visit: $175 (same household)

  • Sick Visit: $155 (30 minutes or less)

  • Extended Sick Visit: $180 (30–45 minutes)

  • Chronic Condition Follow-Up Visit: $180 (45 minutes)

  • Telemedicine Visit: $100

  • 15-Minute Phone Consultation: $45

  • 30-Minute Phone Consultation: $75

Well-child visits include developmental screening, growth measurements, vision screening for children over 4 years old, and hearing screening.

  • Rapid Strep Test: $15

  • Rapid Flu Test: $25

  • Rapid RSV Test: $25

  • Rapid COVID Antigen Test: $25

  • Rapid Urinalysis: $15

  • Stool Hemoccult Test: $25

  • Foreign Body Removal: $25

Enrollment Fees

$150 one time only enrollment fee per family

Enrollment fees are non-refundable.

Additional Information

  • A $20 per-child surcharge applies to families living more than 30 miles from the 85016 Phoenix ZIP code.

  • HSA funds may be used for eligible visits, and families may submit qualifying expenses for FSA reimbursement.

  • Superbills are available upon request for families seeking reimbursement through out-of-network insurance benefits.

  • Direct provider texting during extended hours and unlimited visits are not included in this plan. Families seeking these benefits may prefer our All-Inclusive Membership (AIM).
